I recognize that the Silver Foxes Seated Boot Camp class requires physical exertion that may
feel strenuous at times. I understand that it is my responsibility to consult with a physician prior to and regarding my participation in the above mentioned class. I represent and warrant that I have no medical condition that would prevent my participation in the program. I will let my instructor know if I am taking any medications that may alter my abilities to participate in this class.


I agree to assume full responsibility for any risks, injuries or damage know or unknown which I might incur as a result of participating in the program. Such injuries may include, but are not limited to, heart attacks, muscle strains, muscle pulls, muscle tears, shin splints, heat prostration, or any other illness or soreness. I knowingly, voluntarily and expressly waive any claim I may have against the Beyond The Push Fitness for injury or damages that I may sustain as a result of participating in this class.


I have read the above waiver and release of liability and fully understand it contents. I voluntarily agree to the terms and conditions stated above. 

*** For this class, you will need...
-> A sturdy chair (armless preferred) or wheelchair (locked position)
-> A light set of dumbbells (1, 3 or 5 lbs) OR substitute ( 2 full water bottles, 2 canned food items, 2 bags of beans, etc.)
-> Your water bottle
